The Team Implementation Checklist guides the development, implementation,
monitoring and revision process for building a positive school wide culture.
The team managing the positive behavior support goal completes the Team
Checklist at the beginning of PBS implementation and uses it at
regular intervals thereafter (monthly or quarterly) to monitor progress.
The first section of the Checklist, Startup Activities, is primarily
for initial implementation monitoring, although it can also be used at any
time to re-assess the status of implementation. The second section, Ongoing
Activities, provides the team with a tool to evaluate the
activities required to sustain a PBS system.
Team Checklist information is used to monitor the progress of the entire
initiative and to help sustain efforts across time (years) as well as through
administrative and staff changes. Data is summarized and graphed to show
progress in each of nine areas addressed by the Team Checklist, and these
results are used to create an Action Plan. The Action Plan provides a more
detailed organizational structure for team management of the PBS-related School
Improvement Goals. During the school year, the Team Checklist is used to
track achievement of the major tasks defined by the School Improvement
Goals.

Instructions for Completing the Team Checklist
Who should complete the Checklist? The appropriate group
to complete the Team Checklist is the team that manages the positive behavior
support goal. Each checklist is completed with members of the entire team
providing input on each item.
When should Checklists be completed? At the beginning of
the school year or at the initial onset of the PBS project, Team Checklist
results are used to prepare the Action Plan for the year.
Afterwards, completion of the Checklist on a monthly or quarterly schedule will
enable the team to track achievement of the major tasks defined by the School
Improvement Goals. The team decides the most useful time line by determining
the cycle for using the data.
How much time is required? The Team Checklist takes
about 10 minutes to complete and is completed by the team on a quarterly or
monthly basis during the academic school year.
What steps should be followed? Follow the steps
below to complete the Team Checklist:

The team facilitator, as part of a team meeting, reads each checklist item and
records the consensus of the team with an ‘A’ for achieved, an ‘I’ for in
progress, and an ‘N’ for not started.

As conversation about each item is generated, the facilitator records the
anecdotal comments and keeps the meeting focused on the Team Checklist. This
strategy sets the stage to revisit additional comments later in the meeting.

The team may use only the Startup Activities section, only the Ongoing
Activities section, or both. If a section is used, all questions in the
section must be answered.

The completed Team Checklist should be sent to the district/region/state PBS
Coordinator for summarizing and monitoring.
Using Team Checklist Results
Team Checklist results are used to adapt action plans to school positive
behavior support needs by monitoring the implementation and maintenance
of PBS systems. Checklist results thus help determine the
most effective allocation of faculty, financial, and other
school resources.
The Overall Implementation report
and charts can be used to monitor progress over time. Pay attention to
change in the percent of items implemented as well as to the overall
percent implemented and partially implemented scores. The purpose of this
chart is to assist the team to identify patterns of overall change across
months.
For specific action planning, use the
Implementation by Feature report and chart. The chart illustrates
the extent of implementation of nine features considered critical to the
success of positive behavior support systems.
